Updates in our understanding of local anaesthetic systemic toxicity: a narrative review

چکیده
Despite advances in clinical practice, local anaesthetic systemic toxicity continues to occur with the therapeutic use of anaesthesia. Patterns presentation have evolved over recent years due part increasing ultrasound which has been demonstrated reduce risk. Onset is increasingly delayed, a greater proportion reports are secondary fascial plane blocks, and cases where non-anaesthetist providers involved. The evolving context presents challenge for diagnosis requires education all physicians, nurses allied health professionals about these changing patterns risks. This review discusses: mechanisms; prevention; diagnosis; treatment toxicity. dose used, site injection block conduct technique important determinants toxicity, as various patient factors. Risk mitigation discussed including care at-risk groups, such as: those at extremes age; patients cardiac, hepatic specific metabolic diseases; who pregnant. Advances landscape novel applications settings anaesthesia also described. Finally, we signpost future directions potentially improve management utility anaesthetics remains unquestionable thus maximising safe appropriate drugs should translate improvements care.

Local anaesthetic systemic toxicity following oral ingestion in a child: Revisiting dibucaine
Dibucaine, a potent and toxic local anaesthetic, although currently withdrawn by the United States Food and Drug Administration for use as a spinal anaesthetic, continues to remain available in many over-the-counter topical formulations. Systemic toxicity following oral ingestion of local anaesthetics is rare. We report a case of accidental ingestion of dibucaine (ear drops) in a 7-year-old chi...

It is now 10 years since the first case reports of the use of intravenous lipid emulsion (ILE) as treatment for local anaesthetic systemic toxicity (LAST). In spite of very scarce data on efficacy, safety and mechanism of action, this novel therapy was enthusiastically adopted by anaesthesiologists, clinical toxicologists and emergency medicine physicians worldwide. However, recent thoroughly p...
متن کاملCurrent evidence supports use of lipid rescue therapy in local anaesthetic systemic toxicity.
Readers may be surprised by the recent editorial of Dr. Rosenberg, ‘Current evidence is not in support of lipid rescue therapy in local anaesthetic systemic toxicity’. Lipid rescue therapy (LRT) is widely accepted and recommended for treating local anaesthetic systemic toxicity (LAST) but Dr. Rosenberg purports that the efficacy and safety data are insufficient to warrant its use for this indic...
متن کاملCardiac Arrest after Local Anaesthetic Toxicity in a Paediatric Patient
We report a case of a paediatric patient undergoing urological procedure in which a possible inadvertent intravascular or intraosseous injection of bupivacaine with adrenaline in usual doses caused subsequent cardiac arrest, completely reversed after administration of 20% intravenous lipid emulsion. Early diagnosis of local anaesthetics toxicity and adequate cardiovascular resuscitation manoeuv...
متن کاملThe toxic impact of local anaesthetics in menopausal women: causes, prevention and treatment after local anaesthetic overdose. Local anaesthetic systemic toxicity syndrome
Gynaecologists often use local anaesthetics in their medical practice. Some concomitant diseases during the menopausal period may cause problems during the qualification of postmenopausal women for general anaesthesia in gynaecological surgery. Many authors suggest the application of local analgesia for particular kinds of gynaecological surgery procedures performed on postmenopausal women, tak...
